  • Understanding why people die is important. This knowledge will inform what sort of healthcare is needed, to allow people to live longer, e.g., another heart hospital or a cancer center, better care in PHCs
  • The current quality of death notifications is poor. Death notification is technically difficult, and requires input from multiple people.
  • We are currently engaging stakeholders to create a simpler, more robust death notification form, which will in due course be entered electronically only. You can download the link to the current version of the notification 07-05-24 Death Notification 17'00.xls.
  • Any comments, in particular improvement suggestions are most welcome: write a [comment] or [edit] the form

where would the death of a new born be documented ?
on time line - Corniche hospital not included ?
